Fountain Life operates at the intersection of advanced imaging, genomics, and AI to fundamentally reshape healthcare from reactive to proactive. Dr. Kapp, an orthopedic surgeon and former Air Force flight surgeon, explains how his company scans members with AI-powered imaging and genomic sequencing, collecting approximately 240 gigabytes of data per person across 15+ billion clinical data points. Key findings reveal that 88% of their 10,000-member population shows some form of coronary disease, with 14% having moderate to severe disease while completely asymptomatic - highlighting the failure of traditional symptom-based medicine. Fountain Life's proprietary AI system, called Zori, processes this data in 2-5 minutes and surfaces patterns that human clinicians might miss, enabling treatment recommendations and early interventions. The company's approach leverages lifestyle medicine, precision pharmacology, and continuous monitoring to reverse stage-zero cancers, halt cognitive decline, and arrest heart disease. For entrepreneurs, optimal health translates directly to mental clarity, energy, and productivity - the core assets needed for sustained career performance. Kapp argues the healthcare industry's $5.3 trillion annual U.S. spend is ripe for disruption through early detection and prevention, positioning Fountain Life to scale from high-income populations toward mainstream access within 3-5 years as technology costs decline.

Key takeaways

  • →88% of Fountain Life's members show coronary disease with 14% having moderate-to-severe blockages while completely asymptomatic, demonstrating that traditional doctor assessments miss critical early-stage disease.
  • →Zori, Fountain Life's AI system, processes 240 gigabytes of personal health data in 2-5 minutes and surfaces disease patterns that would take physicians days to manually review, enabling earlier interventions.
  • →Entrepreneurs who optimize health - monitored hormones, gut health, cognitive function, and physical markers - maintain peak mental acuity and productivity into their 90s, directly impacting business performance and longevity.
  • →Early-stage disease detection and lifestyle medicine interventions (diet, targeted pharmacology, continuous monitoring) can reverse type 2 diabetes, heart disease, and cognitive decline without waiting for breakthrough therapies.
  • →The shift from reactive, symptom-based medicine to proactive, continuous health monitoring with AI will lower treatment costs dramatically and give individuals genuine agency over their health outcomes for the first time.

Questions this episode answers

What percentage of healthy-looking people have hidden coronary disease?

According to Fountain Life's data from 6,000 people who underwent advanced cardiac AI scans, 88% of their average 52-year-old member population shows some form of coronary disease, with 14% having moderate or severe blockages while remaining completely asymptomatic.

How does Fountain Life's AI system Zori process health data?

Zori processes approximately 240 gigabytes of personal data - including whole genome sequences, 10,000 MRI images, and all clinical reports - in 2-5 minutes, identifies patterns across 15+ billion data points, and makes treatment recommendations to physicians.

Can you reverse heart disease, type 2 diabetes, and cognitive decline?

Yes, according to Dr. Kapp, all three can be reversed today using a combination of lifestyle medicine (primarily plant-based diet), targeted pharmacology based on individual cholesterol metabolism, and continuous monitoring - though these approaches are not yet widely taught in medical school.

Why do entrepreneurs need health optimization, not just general fitness?

Optimized health directly improves mental clarity, energy, and productivity - the core assets entrepreneurs need for sustained performance - and enables building a long, robust entrepreneurial career into one's 90s without chronic disease-related downtime.

What makes Fountain Life's approach different from traditional cardiology?

Instead of relying on population-based risk algorithms or waiting for symptoms, Fountain Life uses AI-powered imaging and genomics to detect disease at stage zero or one, then uses personalized lifestyle and medical interventions to reverse disease before it becomes symptomatic.

Insight Density

8 / 20

A handful of genuinely interesting data points emerge (88% coronary disease prevalence, 14% severe-and-asymptomatic from 6,000-person cohort, 240 GB per patient), but they're buried under heavy sponsor reads, motivational platitudes, and repetitive pro-health messaging that pads a 27-minute runtime significantly.

88% of the population has some form of coronary disease
14% of people have moderate or severe coronary disease... and are totally asymptomatic walking around

Originality

6 / 20

'Health is the new wealth,' 'proactive vs reactive healthcare,' and 'longevity for entrepreneurs' are thoroughly well-worn themes in 2024 biohacking/longevity discourse; the aviation maintenance analogy for preventive health is a recycled metaphor, and no genuinely contrarian or first-principles argument is advanced beyond the standard longevity company pitch.

it seems like the harder I work, the luckier I get
you wouldn't get in an aircraft and fly across the Atlantic unless the pilot had done all the preventive maintenance

Guest Caliber

11 / 20

Dr. Kapp has legitimate practitioner credentials - orthopedic surgeon, Air Force flight surgeon, multi-venture healthcare operator - and runs a real company with real data; however, the episode is explicitly a paid sponsor placement ('a big thank you for sponsoring today's episode goes to Dr. Kapp'), which constrains candor and turns the appearance into a company pitch rather than an honest operator debrief.

I've been in healthcare for about 30 to 35 years
I owned a group of hospitals at one point. I run surgery centers, imaging centers, medical records companies

Specificity & Evidence

11 / 20

The episode delivers better-than-average specificity for the format - named AI system (Zori), a 6,000-person cohort study with percentage outcomes, a vivid single-patient case study (91% blockage, marathon runner), and industry-scale dollar figures - but lacks published citations, and several claims (Buck Institute reference, democratization timeline) are asserted without supporting data.

we found that he had a 91% blockage in one of the main areas of his heart. And totally asymptomatic
we've got a little over 15 billion data points that we're analyzing with artificial intelligence

Conversational Craft

4 / 20

The host asks entirely generic, pre-scripted questions ('that's a great question,' 'that's powerful'), explicitly endorses and promotes the guest's commercial product mid-episode, and provides zero pushback, probing follow-ups, or challenge to any claim - functioning as an infomercial moderator rather than an interviewer.

I highly endorse it. I love what they're doing
that's a great question. And I think, you know...

com. highlevelfire.com. Bill, say what's up to Fire Nation and share something that you believe about becoming successful that most people disagree with.

Yeah, well, thank you for the opportunity to speak to you guys. Yeah, I think there's a lot of that. I mean, a lot of people want to know. I think probably the biggest thing I find is I hear people talk about luck with being successful.

And I always joke with people, I say that it seems like the harder I work, the luckier I get. So I think that that to me is the one. And I think the other big misnomer is that for some reason, people think that you need to have gone to like a tier one Ivy League school to be successful. And I find that that's patently untrue because most of the successful people I know don't have Ivy League degrees behind their names.

Not that there's anything wrong with the education process, but you can start anywhere and you can literally go anywhere you want to go. Fire Nation, we're talking about longevity is the new wealth. And, but I want to talk about how longevity equals ROI. So you say longevity is the ultimate return on investments.

Why should entrepreneurs be focusing on health span, not just lifespan? Yeah, it's a great question. And I think, you know, what people need to be focused on is first of all, you know, I always tell people, you will probably protect your finances and know every detail about your finances. You probably don't know, however, every detail about your health.

And we believe health is the new wealth because no matter where you are in the financial journey or your entrepreneurial journey, the reality is if you don't keep your body at peak condition, you're not going to finish the race very well. And we see this over and over again. People who quote are burned out or people who finish a career, they sacrifice, they leave everything on the field, and then they wind up finding out that they have really debilitated health conditions because they haven't taken care of themselves.

And the idea here is not necessarily to, you know, just cross the finish line with one entrepreneurial success, but leave yourself in enough optimal health so that you are always at the top of your game mentally, physically, and more importantly, give yourself the runway to make it to 95 healthy. And today we can see that across all of our country today as we start seeing people, octogenarians, people like I use the example Sylvester Stallone frequently, He's totally optimized.

He's still acting, directing, producing, and still throwing 50 in the gym on a bench press. So that may not be your optimal goal, but the reality is there are ways today using technology that's available and widely available to get you to 95 healthy so that you can have a very long and very robust entrepreneurial career. Yeah, I think that's a great example. And with Sylvester Stallone, and I actually just yesterday finished the Hulk Hogan documentary.

And that's kind of the flip side of that, where here's this perfect specimen for such a long time, but then at 71, due to a lot of decisions that were made and just in life things as well, he passed away at 71 years old. So I mean, there's these two different paths that we can take, Fire Nation, as we're getting older. And that's where I wanna talk about being proactive versus reactive build. Because let's be real, traditional healthcare is 100% reactive, but you're flipping that model.

How does Fountain Life detect disease years, literally years before symptoms even show up? Yeah, so that's a very astute observation. It's the main reason we formed the company. I've been in healthcare for about 30 to 35 years.

And as a practicing orthopedic surgeon, Air Force flight surgeon, entrepreneur, I owned a group of hospitals at one point. I run surgery centers, imaging centers, medical records companies, all of it predicated on a system today that we have here in the United States largely, and it's certainly the basis for Western medicine, that waits until, that is very symptom-based, meaning when you go to your doctor, you basically get a whole series of questions about the symptoms that you have.

And that worked really well for the first portion of medicine or the historical part of medicine, meaning, you know, symptoms work really well when you have an appendicitis, a broken bone, maybe some other acute illness, infectious disease, all of the symptoms stuff works really well then. The problem is today, because we've doubled life expectancy in the last 150 years, now what's happened is we've surfaced chronic diseases. And these chronic diseases take hold very early, and they last a really long time until they become symptomatic.

And so our focus at Vountain Life was to use the latest AI-powered imaging technologies, genomic technologies, microbiomic technologies and every technology you could possibly imagine to accelerate or i should say to utilize artificial intelligence and advanced technologies to detect disease at its very earliest whether that be a stage zero stage one cancer or a super early heart disease early cognitive decline long before you can't remember your wife's name or where you left your car keys you know there are changes in the brain that we can start to measure and the idea here is number And so we detect this and then we take our Fountain Life members and we put them on a journey and we start to give them the tool set that they can use to be able to guide their health in a healthy direction.

And then on an annual basis, they can check in and on a quarterly basis, we do blood work. But the point being is that, you know, this comes a little bit from my aviation background and work as a flight surgeon and a private pilot, but you know, you wouldn't get in an aircraft and fly across the Atlantic unless the pilot had done all the preventive maintenance. You really don't want the problem at 40,000 feet. The problem is medicine because we're symptom-based and we're dealing with largely chronic disease and it's not symptomatic until it's a really late stage.

We've already put ourselves behind the eight ball in terms of being able to treat these things at an early stage. Plus, for entrepreneurs, you certainly don't want downtime. Downtime doesn't help you in your career progression. But more importantly, as you get older, your chances of chronic disease go up.

So you really want to get in front of it because we know today you can certainly improve your brain age, reverse that. You can certainly lower your biologic pace of aging significantly. You can also reverse heart disease. You can reverse type 2 diabetes.

All of these can be done today, just not commonly taught in medical school. I want to talk about the power of data because you get to analyze, and I mean literally, Bill, billions of clinical data points. That's insane. So what insights are emerging and how is that changing real world health outcomes?

Yeah, another great question. So we at Fountain Life have scanned effectively about 10,000 people have been through our process. We collect on average 240 gigabytes of data on every single person that comes through from an entire 30x whole genome sequence all the way through to 10,000 MRI images head to toe. as we start to quantify this data.

As we've started to amass this massive amount of data, we've got a little over 15 billion data points that we're analyzing with artificial intelligence looking for deep patterns in the data. And one of the more interesting and actually a little bit alarming things that we have found in our data is that our average member is 52 years old, but we've got people as young as 18 and all the way to 95. But as an average age of 52, slightly more males than females, we find that 88% of the population has some form of coronary disease.

But more alarmingly, 14% of those patients, and this is a study that we did with 6,000 people who had undergone an advanced cardiac scan with artificial intelligence, looking at how much plaque they had in their arteries. We found that in that 6,000-person cohort, we found that 14% of people have moderate or severe coronary disease. Wow. And are totally asymptomatic walking around.

As a matter of fact, this happens on a weekly basis in our centers. A very good friend of mine recently came through testing. He's a marathon runner. He's a plus one golfer.

Great guy. Looks the part. You know, he's perfectly healthy and has been healthy his whole life. His doctor says, you're perfectly healthy.

You're a cholesterol profile. Everything is great. You don't have anything to worry about. And we were just having a conversation.

I said, well, you know, our data would suggest that you should get checked. Not that we see anything overt, but these are the hidden things. And when he came in, we found that he had a 91% blockage in one of the main areas of his heart. And totally asymptomatic.

Now, that's exactly the type of person that's out running one day or is boarding an airplane and drops dead of a heart attack. And we know that half of the people who have their first heart attack don't survive. So your doctor never sees them because they never make it to the emergency room. But more importantly, we know that 70% of people who have a heart attack probably won't even have any symptoms prior to having their first heart attack.

So the point is, even the algorithms that we use, and these are population-based risk algorithms to determine who needs what cardiac interventions, are largely, according to our data, not particularly accurate. And I always say, you know, population health is great for the masses, possibly. However, if I'm the one that's involved, I don't want to be on the tail end of the 5% curve. I want to know exactly what's going on.

And ultimately, what I really want to know is what can I, I want a full assessment of what's going on inside my body so that I don't have to worry in the middle of the night if I wake up with chest pain, because it's probably possibly a bad burrito rather than actual heart issues. That's powerful. Now, what is the plan for that individual with 91%? Like, what can he do to maybe fix that?

Yeah, so we worked with a very advanced sports cardiologist, and we were able to put him into some very advanced testing to find out exactly what his blood flow looked like around that heart, even at that level of stenosis. And while most people would have probably, you know, traditional cardiologists might have put a stint in that location, we were able to actually look, see the blood flow around the certain parts of his heart and realize that he probably could be safely treated with medical therapy.

So a combination of two things. We do a lot around lifestyle medicine. That is changing his diet pretty significantly. He was a pretty high-protein meat-protein diet kind of guy, and we actually switched him to largely a plant-based diet.

But in addition to that, we use the standard pharmacologics when we need to. We determined whether or not he was a cholesterol overproducer or an underabsorber. And then we address that properly. So, you know, we'll re-scan him again probably in another six months or so.

But I think medical therapy in him will probably work just fine. Now, in other people, they might require a stent, especially if it's a 95%, 96% stenotic lesion. It doesn't look like one that is going to easily manage medically. Once again, finding these things early is the key, whether it be cardiac disease or cancer.

And I can just say Fire Nation, in my personal experience, it's peace of mind because I went through the whole fountain life the entire day, the concierge day. It was incredible. This is about three, three and a half years ago now. And I just walked away being like, wow, like I got an incredible day and I got incredible results, a full walkthrough afterwards.

And now like when I maybe do have like a little bit of tightness in my chest. Maybe I'm like, well, I definitely know that I'm a hundred percent clear when it comes to those arteries and different things along those lines. Cause they, you know, I went through that whole imaging. So it's just like, okay, I can set peace of mind.

And of course I want to get regularly checked up, but man, it's either getting peace of mind or finding information you need to know, you want to know to make lifestyle changes now to make potential medical changes You need to now before you know You one of those people boarding that plane going on a jog fill in the blank So we going to be talking about AI and precision health health equals performance building the future and more When we get back from thanking our sponsors Fire Nation, if you are building a real business, you need real infrastructure.

Bill, we're back, and I want to talk about AI and precision health. How are AI, advanced imaging, and genomics, how are they coming together to create a more predictive and personalized healthcare model.

We're very excited about where AI can go. And I know there's a lot of controversy around AI. However, I will tell you for what we do and what we think the future of health is, because ultimately, just from a first principles approach, if you were going to redesign an entire healthcare system, you certainly would not design the broken system that we have today. You know, in my mind, the best way to, you know, start if you had a clean sheet design would be we should use advanced technology to detect disease at its very earliest stages.

and treat it or reverse it so it doesn't become a problem late. And when we do that, we solve most of the long-tail problems in medicine today. However, today the way the system is, you know, obviously the scary you are, more money people make. So it's very difficult because we don't pay, unfortunately, doctors to keep you healthy.

Worse yet, we don't give them the tool set at the front to be able to fully analyze the human body. And so what we've been very excited about is AI has come on the scene, and we've been in business a little over five years now. But what we've seen is an evolution of technology that allows us to take this 240 gigabytes of data that would normally take a clinician days to pour through to be able to determine not only all of the radiology reports, but all of the images, all the genetic reports, the microbiome reports, all of the functional testing we do.

It could take a physician a day or two just to process the information. And now with our AI, which we call Zori, Zori has been reinforcement trained over the last three years and has a very deep knowledge base using everything that's publicly available, plus a lot of our private data. And what we can tell you definitively now is that Zori can take all of that data and not only analyze the data and put it into a very comprehensive format in about two to five minutes. In addition to that, it can actually look through the data for patterns in the data and make recommendations for treatment to our physicians for the first time.

And so what is happening now is we're able to bring brand new physicians in that may not be well-versed in longevity medicine, but using the tools like Zori, Zori can help guide them and surface things that otherwise may be overlooked. One of the exciting parts about what we're doing also with artificial intelligence is that ultimately every member at Fountain Life not only has gone through this really deep assessment, but they have an app on their phone with access to Zori. And so they have as much information on their health data as their physician does.

And if they have other health records they want to upload to Zori, Zori will be happy to read those as well and incorporate them. because we believe that knowledge is power and people have never really had an opportunity to have a lot of agency over their health. You know, we refer to the human body. A lot of people look at it like a black box.

I don't know what's going on inside. And so now as we start to educate people, you know, Zori speaks to them in layman's terms so they can understand very complex topics that would be otherwise difficult to grasp because it reduces it down to something that you can really understand. We believe the future is here with this type of medicine. And also going forward, the cost is coming down on literally every single year to the point I believe every American will be able to access their employer in the next three to five years.

As this does this, then all of a sudden we change the paradigm in healthcare from reactive and episodic to proactive and continuous. And because we can identify problems early, the cost of treating things late really does not materialize. So we know it's much, much cheaper and much better for patients to treat things at stage one in early disease states than it is waiting until they're full-blown late-stage cancer or anything like that. So we're excited about this because we think this is going to be the thing that will switch medicine from this very symptom-based approach, reactive, episodic, to this proactive, continuous approach.

Bill, let's talk about how health equals performance because there's a direct connection between optimized health and things like clarity, energy, and leadership effectiveness. Can you expound upon that? Absolutely. I think it is very clear that the leaders that we find in the entrepreneurial space that have the highest energy, the highest mental acuity, and the highest productivity are the ones who actually make large investments in their health.

As you age you going to experience some declines but the reality is if you monitor your systems everything from hormones to gut health you can improve cognitive function physical function and productivity To the point that we know that today, with the tools that we have available, they're not widely known because we don't teach us in medical school, unfortunately. And they're also not widely distributed yet. But with the tools we have available today, you probably can make 95 healthy, according to the latest studies we have out of the Buck Institute.

So the point is we're not waiting for a new breakthrough in gene therapy. We're actually looking at using the tools that we have to put you in optimal physical cognitive function so that you can then be at the top of your game late into your life. And you can start to see this. We talked about Sylvester Stallone, but there are a lot of national and international business leaders that you can see are very optimized around their health.

Let's talk about building the future. I mean, what does it take to build a category-defining company at the intersection of healthcare and technology and longevity? It's a great question. It's been a little bit of a rollercoaster ride.

As you can imagine, you know, I joke with the team all the time. I say, you know, Elon Musk took on the space industry when it was, you know, less than $100 billion industry. And the same for the car industry. The healthcare industry in the United States alone is a $5.

3 trillion spend annually. It's one half of the global spend for healthcare, which the global spend on healthcare is about $10.8 trillion. So to try to take that on is a daunting task.

However, I believe we're at the breaking point in medicine. And so where we are able to gain our energy and our enthusiasm and our persistence comes through the fact that the mission at Fountain Life is to fundamentally reshape how we do health care, not only here in the United States, but worldwide. And we believe that coming out of COVID and coming out of really ever exponentially increasing health care costs, we believe we're positioned as a company to do that. And I think what we're finding after five short years is there's a lot of traction in the market.

We're also very blessed to have the AI tools and the advance of technology marching forward along with us. And as we continue to make that transition, we continue to find businesses and interested individuals and people who want to lower the cost of their health care, but more importantly, really get agency over their health for the first time. So it starts with hiring great people. We have an amazing team at Fountain Life from our chief medical officer, Dr.

Don Musalem, all the way through our technology team, our AI team. We have an unbelievably talented group of physicians and clinicians that really round out the experience. We have focused largely, you know, on the higher income populations because that's kind of where you start. I always tell the teams, you know, you've got to sell a few hundred thousand dollar Teslas until you can sell a Model 3.

But we're in the point of inflection now for the company where we will begin to start to democratize not only the AI tools that we've developed, but also start to democratize access and being able to find lower points of entry into the fountain ecosystem where you can start to find the critical components of testing at a much more affordable rate. because ultimately we're still a cash-based system and we're outside the traditional reimbursement model for a lot of reasons. One is it allows us to move much, much faster, but secondarily, it's gonna take your insurance company a little bit of time probably to get on board with this.

And so our goal is to make this available to as many people who are interested in changing their health and getting on the journey and learning about what's going on inside their body. And then over time, we believe this will become the new standard. Bill, if Fire Nation wants to learn more about what you have going on and maybe they wanna take that next step. What is your call to action for our listeners today?

Yeah, sure. Happy to have you guys. You can look on our website at fountainlife.com.

We have a whole group of program advisors that'd be more happy to take your call and walk you through the different types of programs we have. Once again, we'll be launching our digital program. It's almost a pure digital program in June, which I have a much more approachable price point. But what I think is the first step is education.

We tell people all the time, Your job, number one, as a consumer and as a healthcare, if somebody interested in your health, is to educate yourself, to find out what is available out in the open market today. And be careful about who you listen to. I mean, there's a lot of longevity companies. You know, we are very science-backed and AI-driven.

But what's most important is to make sure that you educate yourself because the tools are out there today. Most physicians are totally unaware of them because they focus on sick care. They don't focus on keeping people healthy. and they're starting to wake up, but I think it's going to be a little longer until they fully wake up.

But I think the first step would be to talk to us at Fountain Life. We certainly have a newsletter you can sign up for, and then certainly you can follow us on any of the social media that we have. Fire Nation, you're the average of the five people you spend the most time with. You've been hanging out with WK and JLD today, so keep up the heat.
